Renewal period begins today. Applications for provider organizations, training institutions, and supervising hospitals are required to be submitted 60 days prior to your renewal date.  The applications can be submitted by mail to 302 W. Washington St., Rm E239 / Indianapolis, IN 46204, by fax to 317-233-0497, or by email to dhscertifications@dhs.in.gov.
